During sewer line replacement work, workers were hand exposing a 2-inch gas line that was cross-bored through a 12-inch concrete sewer main. The gas line ruptured, and natural gas was released uncontrolled for about 1.5 hours before the utility owner stopped the flow and repaired the line. Workers were evacuated from the immediate work area. There were no injuries.

A worker was troubleshooting electrical issues at a newly constructed residential building. To access a ceiling-mounted electrical connection box the worker was in a scissor-lift basket elevated to about 12 feet and positioned under a gate. Another worker activated the gate remotely, unaware of the worker in the scissor lift. The gate opened, and it struck the scissor lift, causing the lift to tip over with the worker still inside the basket.

During placement of concrete to create the fourth level of a suspended slab roof, a failure occurred in the slab forming system. A section of the formwork (about 12 by 12 feet) fell about 2 inches and concrete flowed out at the leading edge of the pour. No injuries were reported.
