An excavator was loading concrete sanitation rings into the back of a gravel truck. A worker who was disconnecting the lifting chains from the sanitation rings was fatally injured.

During the levelling of a formwork table in an underground parkade under construction, an adjustment pin holding one of the shoring posts of the table dislodged, causing one corner of the table to tilt toward the deck below. There were no injuries.

A young worker was working on a 9:12 sloped roof when the anchor they were attached to failed and detached from the roof. The worker fell about 16 feet to the concrete grade.
A worker performing concrete formwork activities fell 12.5 feet from a ladder onto an adjacent benched section of grade, then rolled down a slope to the lower grade level 4.5 feet below.

During work to install new water mains, an excavator struck and damaged an underground propane main line. The utility provider responded and controlled the release after three hours. Several residences and businesses were evacuated.

An excavator was digging a trench 2 metres deep to open up a water main for repairs. A hydrovac was being used to reveal underground utility lines and pipes every 10 feet before the excavator dug the next portion. The excavator struck a gas line (2.5-inch diameter) running perpendicular to the water line, and an uncontrolled release of natural gas occurred for about four hours. The fire department and utility owner responded, and a nearby building was evacuated.

A civil works crew was excavating a trench for new hydroelectric conduit installation under a roadway. During this work two 3/4-inch gas lines were exposed by hand shovel. Believing that these gas lines had been disconnected and been made safe, the crew removed the line. When they did the same to the second line, a gas release occurred. The fire department and the utility provider responded. A large area including a school, commercial workplaces, and the road were evacuated until the natural release was controlled.
