A paving company was conducting chaining (measuring) activities in the slow lane, which had been closed with a traffic control zone. A public vehicle operated by a non-worker collided with the back of a work vehicle with a flashing arrow board in the closed lane. The driver of the work vehicle (a young worker) sustained minor injuries.

A worker was pressure testing a buried 3-inch PVC water main with nitrogen. The worker sustained injuries to the head when a section of the pressurized PVC piping unexpectedly broke free and struck the worker in the head. Pressure escaping from the pipe also disturbed some loose sand/gravel in the area, resulting in this debris striking the worker in the face and entering the eyes.

Two workers were travelling southbound in a pickup truck from a local office back to the company’s head office when their vehicle collided head-on with a small car travelling northbound. One of the workers was seriously injured, and both non-workers in the car died.
