A worker on a ladder was vibrating concrete into a 16-foot-high cardboard column form when the form suddenly collapsed. The worker fell from the ladder.

A worker on a steep-sloped roof (6:12) was moving his fall protection lanyard from a hip D-ring to the back D-ring of his fall protection harness when he lost his balance, slid down damp building paper, and fell 18 feet to the ground.

A rough-terrain telehandler was turning in reverse with an elevated load, on a slight incline (2 to 3 degrees) when the machine overturned. It landed on its side, trapping the operator in the cab. The operator broke the rear window with a hammer and escaped.

Three workers were dismantling a free-standing golf simulator when the walls and ceiling of the golf simulator collapsed. One of the workers was struck and pinned by a wall 10 feet by 15 feet.

An excavator contacted a residential 3/4-inch gas stub line. Gas escaped into the crawl space of the home and was exiting through the sewer system. The neighbourhood was evacuated by the local fire department due to gas level readings found in the affected home.

Two workers were in an excavation running equipment, working to establish the grid, placing crush, and moving lock blocks at the toe of the slope. A small slough occurred. The workers got out of the excavation and then a large collapse occurred.
