A roofer climbed out of an unguarded dormer window to access the roofing worksite. The worker, who was wearing a personal fall protection harness but was not connected to his lifeline, slipped on the icy, wet roofing material on the roof (reported to have a pitch of 7:12). The worker fell 22 feet to compact ground, narrowly missing a steel dumpster.

The young worker came into close proximity to the siding installer as the installer lowered his nail gun. The young worker contacted the gun and was struck in the head by a nail as the nail gun discharged.
