Four workers were moving an MCC (motor control centre) unit weighing about 1,200 pounds to install it in an electrical room. One of the skates detached from the MCC unit and the unit fell to one side and contacted the ground. The other side of the unit then struck and landed on a young worker.

A worker threw a rope tagline, about 20 feet long, from the third level of a building under construction to a second worker at grade level. The rope became tangled on energized low-voltage (240 volts) power lines. The second worker, while standing on a delivery vehicle, grabbed the wet rope and tried to remove it from the energized power lines. Other workers observed a flash and heard a “pop” noise. The second worker was transported to hospital for assessment and released.
