Workers at a residential duplex under demolition noticed the odour of natural gas and contacted emergency services, who responded and determined that neither the gas nor the electrical utility services had been properly disconnected. The utility service providers attended the site and isolated the services.

Half a sheet of oriented strand board struck the back of a worker’s head. The board was dropped by a worker who was above the other worker on a scaffold system. It was reported that the board slipped out of the higher worker’s hands and fell about 3 feet before striking the worker who was bending over. The injured worker was wearing safety headgear.

As a mini excavator was being offloaded from a flat-deck trailer attached to a pickup truck, the pickup and trailer began to roll down the sloped residential street. The operator jumped from the mini excavator, which was still on the trailer, sustaining multiple injuries. The worker later succumbed to these injuries.
