A framer was working on the fourth floor of a condominium under construction. A strong gust of wind pushed a 5/8-inch sheet of plywood into the worker, pinning him to the perimeter guardrail. The guardrail broke and the worker, unable to regain his balance, fell to the concrete walkway 32 feet below.

An operator of a front-end loader was clearing snow on a private road used to access a gravel pit. The loader slid backwards down the steep road before going over an embankment and rolling once. The operator was trapped in the loader and had to be extricated by search and rescue personnel.

Workers were sheeting trusses on a new house. One worker asked another worker to pass him a sheet of plywood. The second worker stepped onto a nearby skirt roof and pried a sheet of plywood off an unmarked skylight opening (4 feet by 4 feet) it was covering. The worker then fell through the skylight opening to the wooden floor 9 feet below.

A lone worker accessed a building roof hatch and started descending the vertical fixed ladder. While descending, the worker fell about 12 feet to the hard ground surface below. The worker sustained multiple injuries and walked out of the building, searching for help. A member of the public assisted the worker by calling for an ambulance.
