Eight workers were placing concrete for the roof of a residential garage (8,000 square feet). About halfway through the pour, the supporting falsework failed, resulting in a total collapse of the poured section of the slab. Workers fell to the slab below.

A telehandler forklift was placing a full package of 2×8 lumber, 16 feet long, onto the tops of second-floor walls when the package broke through one of the top plates. One side of the load fell to the second floor, and the other side pushed the exterior north and west walls outward. The west wall dislodged and fell to the main floor concrete slab below. There were no injuries.
