A worker who was disconnected from a lifeline and anchor assembly removed a perimeter roof jack from the lower of two toeboards. The lower toeboard was originally supported by three roof jacks. After cleaning debris from the elevated toeboard site, the worker stepped back onto the unsupported lower toeboard, which pivoted about the central roof jack. The worker slid down the 14/12 pitch roof, struck the gutter, fell 3.2 metres, then landed feet first on the lawn.
