A young worker was installing an additional circuit to a ventilation panel that panel was live. The worker contacted the 600-volt side of the transformer in the panel, which created an electrical path through the worker’s hand to ground.

Two workers were moving fence panels with a pickup truck. One of them (a young worker) was in the back of the truck and fell out of the moving vehicle.

A pickup truck towing a lighting trailer was reversing when a worker (spotter) noticed that the trailer hitch was not securely connected to the hitch ball. The spotter signalled the driver to stop. The equipment was positioned on a slight incline, and as the spotter tried to correct the hitch connection, the trailer detached from the ball and struck them.

A worker was trying to move a construction fence panel to allow clearance for their skid-steer. Two other connecting panels did not have upper fence panel pins, which caused a panel to swing, pinching one of the worker’s fingers and knocking the worker to the ground. Three loose fence panels then fell onto the worker.

A 5-ton mini-excavator was excavating near an identified underground utility. The excavator struck the high-pressure natural gas service, causing an uncontrolled release of natural gas. The site was evacuated, emergency services and the utility owner were contacted, and the service was repaired.

A utility road crew was performing mechanical digging on a roadway to facilitate placement of a new water line service. While digging, the excavator contacted a gas line (1.25 inches, 60 psi), resulting in an uncontrolled release of natural gas. The immediate worksite was evacuated while the utility owner made repairs.
