A worker was standing on the rung of a suspended slab scaffold frame, moving a piece of lumber (16 feet long, 4×6 inches), when they fell about 6 feet to concrete.

An operator was tipping their cargo box to unload gravel when the lip of the box contacted an overhead line, breaking a utility pole and causing a 14.4 kV power line to fall onto the truck. The operator remained in the cab until the utility owner responded and removed the power lines.

A worker was cutting a polycarbonate sheet on an unguarded table saw. During the cut, the material kicked and one of the worker’s hands contacted the rotating blade.

A worker was conducting excavation work for installation of domestic water lines. The excavator’s bucket damaged a terminated end of a steel 2-inch natural gas line, causing a release of gas.

A worker was operating a rubber-tired loader on a slope of about 45 degrees, compacting a large pile of landfill waste for cover. The loader drove over a soft (uncompacted) area, became unstable, and rolled over down the side of the landfill.

A worker operating a hydro vacuum truck was climbing over a metal park gate adjacent to the excavation when the boom of the truck inadvertently lowered. The worker was trapped between the boom and the gate until another worker raised the boom.

An employer’s representative was not able to locate a natural gas service line (1.25-inch diameter) by consulting the BC 1 Call documentation. When they were also unable to locate it by hand, they tried to remove a top layer of ground using powered equipment. The equipment contacted the line, resulting in a release of natural gas.
