A tower crane had completed lifting a load and the tower crane operator was advancing the trolley towards the mast while hoisting the load block with lifting hook and rigging chains (empty) to allow the crane hook to relocate to another location at the jobsite. The load block, hook, and rigging chains (weighing about 1,000 pounds) fell uncontrolled from a height of about 20 feet, striking the concrete floor surface below. The excess load line spooled onto the same surface. Workers were within the work area, but no injuries were reported.

An excavator was grading the sub-base of a roadway when the bucket hooked two rocks which then pinched and ruptured a 3/4-inch natural gas service line. The police and fire department were on site and businesses in the area were evacuated. The utility company repaired the damaged line.

Workers involved in sorting metal in the lower laydown area of a pulp mill were exposed to air contaminants from a plume of vapour from the effluent cooling towers. Five workers, including one young worker, reported to first aid with symptoms such as headaches, nausea, sore throat, vomiting, and light-headedness.
