A young worker was conducting painting activities from a combination ladder set up on a 5:12 sloped skirt roof. The ladder began to slip on the concrete tile roof surface. The worker jumped backward off the ladder, landing on the paved driveway about 12 feet below.

The leads and boom of a pile-driving crane were laid down so that they could be disconnected. A worker went to flip the wedge socket and shackle over for easier access to the mollies and keeper pin. While doing so, the worker bumped the spreader bar, which was in the upright position. This caused the spreader bar to come down, pinching the worker’s leg between the bar and the lead cross-member.
