During flyform stripping, an unsecured piece of 3/4-inch plywood (filler) measuring 2 feet by 5.5 feet fell from the 25th floor. The piece of plywood struck and broke a window in a building across the street.

A worker could not control a runaway 8-ton shop truck on a steep grade. The worker tried to ditch the truck in an attempt to stop it. The truck rode up an embankment where it rolled multiple times.
