A drill head was being elevated to the top of the mast of a track drill. An air hose apparently became looped around a mast cross member (hose support bulkhead). This stretched the hose, causing the hose fitting and whip check (safety strap) to fail. The hose and fitting fell, striking the head of the worker operating the unit. The worker received first aid at the site and was transported to hospital by ambulance.

Workers involved in metal assembly and welding at a pulp mill’s “Evaporation Expansion Project” were exposed to air contaminants and vapours in the area of the heavy black liquor tanks. These tanks had been leaking and at the time of the incident were in an upset condition, venting vapours. Eight workers, including three young workers, reported to first aid with symptoms of headaches, nausea, dizziness, eye irritation, and burning skin. The workers were transported to the emergency department at the local hospital for treatment.
