A young worker was ascending a ladder to the top of a wall form (16.75 feet high). As he neared the top of the wall form, a piece of equipment he was wearing got caught on a rung of the ladder. His foot slipped from the step of the ladder and he fell 12-13 feet to the work platform below.

A worker was descending from a scaffold via a portable aluminum ladder. The ladder buckled and the worker fell about 6 feet to the ground.

A worker was replacing deck pipe on a concrete pump truck. The truck’s 52-metre boom was not extended but was raised about 10 feet and the outriggers were not extended. The truck became unstable and flipped onto its side, with the raised boom landing on and crushing two parked cars. No injuries were reported.

During bridge repair work, alternating lane closures were in effect. A traffic control person (TCP) was struck by a vehicle that unexpectedly bypassed a line of vehicles being held back by the TCP.

After cutting a section of concrete wall, a young worker began removing the concrete cutting saw from the wall. The cut section of wall, which was being held by a crane, became unstable and crushed the worker’s lower arm between the saw and the wall.

Two workers were on scaffolding, stripping ceiling formwork in the basement of a hospital construction site. They experienced nausea and dizziness just as they were being evacuated in response to a gas monitor alarm for nitrogen dioxide. A third worker (a young worker) had already left work due to nausea. All three received medical attention.
