A worker was about 12 feet up a 20-foot aluminum CSA Type 1 ladder pressure-washing a metal handrail with a 3000-psi pressure washer. Another worker, on the ground, was manually stabilizing the ladder. The top of the ladder began to slip to the right. The ladder then twisted counter-clockwise and fell to the right onto the ground. The worker on the ladder also fell to the ground, landing on his feet and then rolling backward.
