An articulating boom crane was lifting into place an assembly of five second-storey floor trusses. As they were about to be placed on first-floor wall supports, the load broke free from the rigging, falling to the ground and striking the crane operator and a carpenter in the area. |

A worker was installing sub-fascia trim at the outer edge of a roof truss system while standing on the 2×6 lumber top plate. When the worker stepped onto a 2×4 piece of sub-fascia, it broke away and the worker fell 19 feet to the ground due to excess slack in his fall protection lifeline. |

A worker was using a 10-foot stepladder to install pot lights under an upper-level roof soffit about 20 feet above grade. The base of the ladder was about 16 feet above grade, on a 6:6:12-sloped cedar shingle eyebrow roof section and was being held by a worker standing on the eyebrow roof. The ladder slipped out at the base and both workers fell to grade. The worker who had been on the ladder was seriously injured. |

While flying a loaded concrete bucket over a construction office trailer, a tower crane lost control of the load. The bucket dropped 10-15 feet and struck the edge of the trailer’s roof. The bucket then tipped sideways, and dumped half the load of ready-mix into the open excavation below. |
