Three workers were applying a protective coating to natural gas pipelines under construction when a vacuum truck struck the pipes, which in turn struck the workers.

During preparation of a shotcrete mock-up panel (10 by 10 feet), bracing components were removed and a worker placed an extension ladder against the panel. As the worker climbed the ladder, the panel shifted and fell over, striking and damaging a concrete bucket. No injuries were reported.

Workers were setting joist trusses on the top plates of framed exterior walls of the lower level. Three of the walls suddenly fell outward, causing the trusses, and a worker on a scaffold, to fall to the surface below.

A tower crane was moving ganged formwork panels in a blind lift. A worker was holding the chains and lifting clamps away from a panel that had just been lowered. As the chains and clamps were being lifted to connect to another panel, a clamp hooked onto the worker’s pant leg. The worker grabbed the chains and was subsequently lifted about 30 feet above the ground. Another worker saw the elevated worker and signalled the crane operator to stop and lower the worker to the ground. No injuries were reported.

A worker was pushing a piece of lumber through a table saw when one hand slipped and contacted the blade.

A young worker was applying stucco on an exterior balcony while standing on an aluminum sawhorse trestle ladder. Descending from the sawhorse, the worker lost their balance and fell from the balcony to grade, landing on protruding rebar.

Workers were installing a fabricated steel stud channel beam, working from two scissors lifts (stationed at each end of the beam). A young worker below, standing below one of the lifts, was struck when the beam came out of the support pocket at that end.

A young worker was using a circular saw to cut a 2×4 at an angle. The worker had one hand on the handle/trigger and one hand lifting the guard when the saw kicked back and the blade contacted the hand holding the guard.

A worker was unloading debris into a metal disposal bin. The bin had been placed on top of an energized electrical cable. When the worker contacted the bin, they received an electric shock.
