Three workers were standing on a suspended aluminum ladder-jack platform installing an overhead 2-ply, 2×10 beam. The platform was supported by a ladder and jack at one end, and a job-built wooden bearer at the other. The job-built bearer failed and the platform collapsed. The three workers, including one young worker, fell about 8 feet to grade.
