During the operation of the permanent traveller (movable work platform) below the bridge deck, a worker’s head became pinned between the traveller guardrail and a steel cross-brace. The worker was evacuated by high-angle rescue crews.

A worker at elevation was struck when another worker lost control of the delivery hose of a concrete pumper truck during a wall pour. The worker who was struck fell about 8 feet from a 2×10 plank to the ground.

A decommissioned elevator, with the cab removed and the elevator platform weighed down with about 1,600 pounds of counterweights, was positioned on the first level of a multi-level elevator shaft. A worker who needed to move a load of 4×6 wood posts to the top floor removed the counterweights from the elevator platform. As the worker stepped off of the elevator platform it suddenly released from the ground floor and began an uncontrolled rapid ascent. It struck the top floor and the elevator’s designed counterweights landed on and damaged the buffer on the ground floor. No injuries occurred.
