Improvements and changes for worker report of injury (Form 6)

Last year, we shared that WorkSafeBC  improved our online injury reporting form  to make it easier for workers to report injuries online — anytime, anywhere. This is part of our commitment to offer faster, more responsive, and convenient service.  

The improved online injury reporting form is for workers with a physical injury from a single incident at work. The questions are easy to understand, the mobile-friendly design means it can be completed on any device (such as a phone, tablet, or computer), and it saves automatically so workers can complete it at their own pace.  

What’s  changing?  

As part of this modernization, WorkSafeBC is also transitioning away from paper-based (fax or mail) worker injury reports:

  • Starting March 1, 2026, the paper version of the worker injury report form (Application for Compensation and Report of Injury or Occupational Disease — Form 6 PDF) will no longer be available on worksafebc.com for reporting injuries by fax or mail.   
  • Instead, workers should submit their injury reports online at  worksafebc.com/report-injury at any time, or by calling the Teleclaim team (Monday to Friday, 8 a.m. to 6 p.m.).   
  • Starting March 2026, if a worker is unable to submit their injury report online or by phone, they can call or email  the Claims Call Centre team for support (for email, select “General claims and injury inquiries”).  

Helpful information 

Workers can report injuries online 24 hours a day, 7 days a week; this is their  first step to start  their  claim, and also to help employers who have not yet submitted a Form 7 receive a pre-filled Form 7 online (where some fields contain information the worker provided).

Free resources available
Employers can also order free  wallet cards  and  posters  about how to report an injury. Both have a QR code for quick access to the injury reporting page. The card fits easily in a wallet or phone case, and the poster is a useful reminder  to display in  key areas like  first aid rooms or lunchrooms.
